For 3b), please based on your past encounters with the clinic/specialty to estimate the duration of the whole medical consultation process. Together with reasonable travelling time, you can come up with a tentative return time. If it turns out to be later (earlier), please phone back to office

For 5), This is the general guideline of ensuring time-off being properly accounted. In this sense, staff should not disappear from the workplace during office hours without reasonable justification. In your example, if the average travelling time from office to the clinic is >30 min, it sounds meaningless to arrive office @8:45 and travel to the clinic for an medical appointment @9:15.

As mentioned in point 1 below, staff is required to arrange as much as possible the schedule of appointment be in early or late in the morning/afternoon session. This help save time of both the staff and the organization.

For your upcoming medical appointments, please 1) provide your tentative return time; 2) indicate if you will report work before/after the appointment, esp. for 25-Nov.

For the QEH appt, normally it will take 2-2.5 hr (waiting time + consultation time) + 45 min travel between office & QEH

• 12 Nov 2009 14:30 Cardiac appt @ QEH -- (12 Nov 2009, I will return office on 13 Nov 2009 am)
  Approved. Your absence for medical appointment should be covered by a half-day sick leave.

• 25 Nov 2009 11:00 Antenatal appt @ private clinic -- (25 Nov 2009, I will report work before appointment, and back after lunch)
  Pending. Please advise your leave time from office for attending the medical appointment, i.e. Appointment scheduled at 11:00. Will leave office at XX:XX and return at 14:00.

• 11 Jan 2010 09:15 Antenatal appt @ QEH -- (11 Jan 2010, I will return office after lunch)
  Approved. Your absence for medical appointment should be covered by a half-day sick leave.

Please note that procedurally, the type of leave which will be covered by the absence for medical attendance would be determined after the medical treatment, examination and consultation.
